  • This project aims to enhance quality end-of-life care in client\u2019s home and in the community and improve the quality of life of end-of-life patients and their family members.<\/li>\n<\/ul>\n

    Project Objectives<\/h3><\/div><\/div>[vc_column_text el_class=”txt fs18″]The Project aims to allow clients to spend quality time with their loved ones in a familiar place and to reduce or avoid unnecessary hospital admission.[\/vc_column_text]

    Target Service Users<\/h3><\/div><\/div>[vc_column_text el_class=”txt fs18″]\n
      \n
    • Clients who are aged 60 or above, with terminal cancer \/ other chronic irreversible illnesses or with life expectancy usually not more than 6 months<\/li>\n<\/ul>\n

        • Facilitation of Advanced Care Planning (ACP)<\/li>\n
        • Family carer support through skill transfer and emotional and spiritual support<\/li>\n
        • Bereavement care<\/li>\n<\/ul>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n
