Project Introduction<\/h3><\/div><\/div>[vc_column_text el_class=”txt fs18″]<\/p>\n

\u201cHospice in Family\u201d Home Care Support Services seeks to provide end-of-life home care service for terminal patients and their family members, in order to preserve their dignity and to improve their quality of life. This project also aims to raise public awareness of end-of-life home care service, and to create \u00a0a positive attitude towards death.<\/p>\n

Project Objectives<\/h3><\/div><\/div>[vc_column_text el_class=”txt fs18″]<\/p>\n
    \n
  • To treat every end-of-life patients with dignity and respect, in familiar surroundings, and in the company of his \/ her loved ones and family members.<\/li>\n
  • To assist, care and comfort end-of-life patients and their family members, \u00a0\u00a0and meet their physical, psychological, social, spiritual and daily living needs.<\/li>\n
  • To enhance mutual support among patients, their family members and volunteers.<\/li>\n
  • To educate the general public about home palliative care and hospice care service and inculcate a positive attitude towards death.<\/li>\n<\/ul>\n

    Target Service Users<\/h3><\/div><\/div>[vc_column_text el_class=”txt fs18″]<\/p>\n

    • Senior patients who are in the terminal stage of their illness and\u00a0aged 60 or above (Transfer from palliative care service of New Territories West cluster)<\/li>\n
    • Social service organizations, schools and the general public in Tuen Mun, Yuen Long and Tin Shui Wai District<\/li>\n<\/ul>\n
