Project Introduction

Provide comprehensive end-of-life care to the patients at the RCHEs

Project Objectives

Through the professional team established under the project, we support and assist subvented RCHEs. The project collaborates with the RCHEs together to promote palliative and end-of-life care services, and develop a model for end-of-life care services in residential homes, so that patients participating could receive appropriate and holistic care

Target Service Users

End Stage/Terminal ill residents in participated subvented RCHEs in Kowloon East, their family members and staff of RCHEs

Service Components

  • Emphasis on Communication, Encouragement and Respect for Residents’ Self-Determination: The project emphasises maintaining good communication between patients and their families and the project team. It encourages participation in the planning and implementation of the ACP, allowing the patients to participate in relevant medical decisions
  • Collaboration Among Different Parties, Continuous Assessment and Alleviation of Residents’ Discomfort: The project team collaborates with the RCHEs and Hospital Authority’s CGAT team, establishing strong communication channels and close collaboration with community doctors and nurses subsidized by the project to provide appropriate care plans for patients. This includes medical support for end-of-life care services, such as health assessment and symptom control. Through continuous assessment, the project provides appropriate and high-quality nursing care to the patients, alleviating discomfort, addressing their psychological, social, and spiritual needs, improving their quality of life, and helping them fulfill their wishes
  • Supporting Families and Caregivers: Through various activities and sharing sessions, the project enhance families’ and caregivers’ understanding on palliative and end-of-life care services, helping them to prepare for and cope with the passing of loved ones, accessing relevant information, cherishing the time spent together. In addition, timely grief care and counseling services are provided
  • End-of-Life Care: When patients participating in the project entering their final moments, professional team and the RCHEs staff will continue to provide the most comfortable care. Suitable patients will be assigned to stay in a specially-equipped “End-of-Life Care Room”, where community doctors and nurses will provide services subsidised by the project, and the patient’s family and friends could be there by their side; allowing patient to pass away peacefully
  • Provide Training for Staff and Promote Death Education: The project also provides end-of-life care service training, on-the-job guidance and professional consultation support services to frontline and professional staff in RCHEs, helping them to master and utilise relevant knowledge and skills, and working together to promote high-quality end-of-life care services within the caring homes
