Abstract
As contemporary models of bereavement have become more nuanced and empirically informed, so too have the practices available to grief counselors and therapists. This two-day workshop offers in-depth training in several of these techniques, nesting them both within the therapy relationship and in the context of current theories and research that provide flexible frameworks for intervention. Making extensive use of actual clinical videos as well as how-to instruction in the use a numerous therapeutic tools, we will discuss and practice several methods for helping clients integrate the reality of the loss into the ongoing story of their lives, while also reconstructing their continuing bond to their loved one.

With the growing of ageing population, end-of-life care is an important social issue to address. The JCECC Conference on Collaboration in Creating Compassionate Holistic End-of-Life Care for the Future (安寧照顧研討會:共建美好的全人晚晴照顧) is the first international conference organised by the Jockey Club End-of-Life Community Care Project (JCECC), which aims to share and discuss the latest research and best practices both locally and internationally to create a holistic end-of-life care system and to provide a platform for wisdom exchange and skill sharing among healthcare professionals, staff from residential care home for the elderly, patients, family members and other interested individuals. Eventually, we hope the conference can help to promote the quality of end-of-life care, enhancing the capacity of service providers, as well as raising public awareness in end-of-life care.

Communication is an important bridge to understand, respond and meet the physical-psycho-social-spiritual needs of the patients. Since discussing end of life issues is still a taboo for many Chinese, a mixture of verbal and non-verbal communication can reduce the resistance in end of life preparation and enhance patients’ quality of life. Workshop on Addressing Moral Distress in End-of-Life Decision Making
Moral distress is described as a phenomenon in which one knows the right action to take, but is constrained from taking it. This phenomenon is not uncommon among healthcare professionals facing end-of-life challenges. What is moral distress? Who is vulnerable? How is it recognized? What are the barriers? What can be done about it? The workshop will help participants explore all these questions to a deeper understanding as well as explore means to effect change.
Workshop on Clinical Ethics and Advance Care Planning (ACP)
Discussions on end-of-life care from the perspective of clinical ethics can facilitate positive outcomes in improving the quality of care provided. Early engagement of patients with a life limiting condition in advance care planning can improve patient and family satisfaction. This workshop will provide a platform for participants to understand these two interrelated concepts and to explore an appropriate approach in the provision of End-of-Life care.

Patients with advanced or incurable cancer often express the need for professional help to cope with their complex situation. Furthermore, the support of health care professionals may often be dominated by symptom treatment and its effectiveness and less on the kinds of needs that patients and families consider most important. Distress is a significant problem for individuals who are living with cancer. Responding effectively requires therapeutic sensitivity and skill. This workshop will discuss a model that draws on elements of therapeutic presence and the integration of therapists’ personal characteristics to provide effective communication and psychosocial care of patients who are experiencing cancer-related distress.

Couple based interventions at the end of life may identify gaps in communication about end-of-life issues; facilitate a sharing of new understandings; reduce levels of distress; provide an opportunity for relational growth; while preparing the partner for bereavement. Participants will be introduced to an intervention that was developed and piloted using the Patient Dignity Inventory (PDI) as the focus of a 1-hour clinical interview. The intervention may be an important adjunct in helping to prepare the couple for the challenges inherent toward the end of life. The focus of the clinical interview around the PDI provides a structure which may be particularly acceptable for men.

This professional symposium is designed for health and social care professionals who are interested in understanding different community end-of-life care options. Experts in end-of-life care from the Haven of Hope Sister Annie Holistic Care Centre, Society for the Promotion of Hospice Care, and the Hong Kong Association of Gerontology will share their rich practical experiences that facilitate “choices” beyond caring at hospitals. Bereavement care is for supporting those who are affected by the death of their family members or close friends. It is an integral part in end-of-life care. Widowhood is a common life experience in older population. This two-day workshop targeted at health and social care professionals who will use bereavement group in supporting bereaved persons. It will introduce the Dual Process Model (DPM), a leading theory in the field, and the evidence based group intervention developed based on this model. Participants will learn about the design, the practical skills and the impact of this intervention model applied in the Hong Kong Chinese context.

Demoralization is a mental state of low morale and poor coping associated with hopelessness-helplessness, meaninglessness and pointlessness, which potentially leads to a desire to die. It is distinct from depression, but can occur co-morbidly with adjustment and depressive disorders. This half-day training workshop is specially designed for health and social care professionals who are interested in demoralization. It will provide an overview on demoralization in end-of-life care settings and risk factor associated with demoralization. Participants will be invited to review clinical vignettes to determine optimal diagnoses and management plans. The innovative Meaning and Purpose (MaP) Therapy will be introduced to help patients at end-of-life to sustain focus on living, identify the many sources of meaning and fulfilment, and thus better negotiate through advanced illnesses.

Screening of family functioning helps discern which families are dysfunctional or in greater need. Advance care planning (ACP) is a process of planning for future medical care in the event that an individual is unable to make his or her own medical decisions. ACP assists an individual in preparing for a sudden unexpected illness or injury, as well as death and dying.
