Abstract: The National Advance Care Planning (ACP) programme in Singapore was launched in 2011 and is modelled after the Respecting Choices paradigm. In this seminar, key findings and lessons learnt from a nation-wide mixed-methods evaluation of the programme will be discussed, highlighting the need for a transformation of health systems, cultural and behavioural norms to enhance its practice and receptiveness in acute and community care settings. To ensure the sustainability of ACP, a patient- and family-centred model of care is required. Drawing from the empirical evidence from a randomised control trials on the Family Dignity Intervention (FDI) for advancing holistic palliative care, clinical examples of how organic end-of-life care discussions can be promoted through a structured narrative reminiscence and life review approach will be shared.
