A New Page in 2024 – JCECC: Unison

After years of effort, the Project has successfully implemented and established an effective medical-social collaboration model for care of elderly. Coupled with the Project’s community education and the government’s focus on end-of-life care, public awareness and acceptance of end-of-life care have gradually increased. Recognising the additional challenges faced by people with disabilities in facing end-of-life care, the Hong Kong Jockey Club Charities Trust conceived and funded a five-year Project in 2024: “JCECC: Unison” aiming to adapt the existing community-based end-of-life palliative care service model to meet the unique needs, and to develop an adjusted new service model so that the patients with disabilities and their caregivers can receive the same high-quality community end-of-life care.

“Joy in Life, Peace in Death – Building A Future of Choice”—the JCECC: Unison Project has consistently witnessed the practice of this belief. We will continue to deepen our community care network, using the innovative medical-social collaboration model to allow every life to write its own fulfilling chapter with warmest support.
